HMO plans are usually going to be your state based plans. They will not cover you across state lines unless there is a medical emergency. The caveat with that is your insurance company gets to deem what is and is not a health emergency. There are also requirements for you to stay in network with your care, meaning less flexibility for you.
PPO plans give you coverage nationwide and they also give you the flexibility to go in or out of network, however, staying in network will always be more beneficial to you. With PPO plans, you also do not need a referral from your primary care provider to see a specialist. You have the freedom to see any specialists on your own accord.

Health insurance premiums are calculated based on a few factors.
On the private health insurance market, there are plans that are health-based, also known as medically underwritten. So the main factors to get on a healthy plan would be your age and your health history. Rates and coverage on the private market tend to be more competitive and comprehensive because the risk pool is much smaller in comparison to Obamacare, which accepts any and every pre-existing condition.
On the public marketplace/ACA/Obamacare, your zip code, age, and household taxable income are all factors considered. Also, your coverage needs will dictate your monthly premium. If you have higher usage, you may consider getting a gold plan which is more expensive versus if you just need a plan for the "what if", then you may just need a bronze plan which tends to have a lower premium.

There are several advantages to working with an agent/broker because there are plans on the private health insurance market that aren't visible to the public unless you're working with a specific agent. So by working with someone, you have access to a broader spectrum of health insurance plans. Private health insurance also tends to make the most sense, especially if you're an entrepreneur/small business owner.
I can also work as a broker for the ACA which means helping educate you on plans that would best suit your specific needs and enroll you into said plan. I would also be able to advocate and assist with any possible future claims.
Can you shop the public marketplace and enroll yourself into a plan? Absolutely. But it helps to have someone in your corner, especially with no extra cost.

Yes! There are plans on the private health insurance market that are offered year-round and are month-to-month. There is also flexibility on start-date as well. Your policy does not have to start on the first of every month.
There are also specific life events that can occur that will give you a 60-day special enrollment period for Obamacare plans. Some examples include moving, lost or losing coverage, change in household size, etc. So you have 60 days after your specific life event to enroll in an Obamacare plan.
